Abstract

Heel reconstruction, caused by pressure ulcers with loss of substance loss, involves a reconstruction with surgical procedures, which provide an adequate covering of skin and soft tissues with the use of fasciocutaneous flaps. In the present case, the use of the Cone Flap is a technique that allows significant results to be achieved to restore the functionality of the affected area, by performing immediate treatment and timely maintenance, providing a dermo-fatty mattress to the affected area, which It significantly reduces subsequent sequelae. The Cone Flap is an idea developed in the Plastic Surgery Service of the “Hospital del Trabajador” in Santiago de Chile, by Dr. Wilfredo Calderón in 1987. Anatomical dissection of the legs of 10 fresh cadavers was previously performed, after injection of blue methylene and barium in the popliteal artery, to check its irrigation. They performed a geometric analysis of V-Y advancement to demonstrate how tissues can advance in this type of flap. This paper reports the case of a 65-year-old male patient, with a history of pressure ulcers in both heels, who attended the University Social Security with necrotic areas in both affected areas.
