Abstract

Introduction: Rhinophyma is a nasal skin deformity resulting from advanced forms of rosacea; its surgical treatment poses a reconstructive challenge due to the need to achieve adequate skin coverage and a satisfactory aesthetic and functional outcome. Regenerative therapy using an autologous membrane composed of nanofat and fibrin-rich plasma could promote healing and tissue regeneration.

Objective: To demonstrate the utility of an autologous membrane of nanofat and fibrin-rich plasma as a temporary dressing for post-surgical skin regeneration in patients with severe rhinophyma, highlighting its favorable impact on the healing and nasal epithelialization process.

Material and Methods: Two cases of male patients diagnosed with severe rhinophyma characterized by marked nasal deformity and excess fibrous and sebaceous tissue, resulting in aesthetic and functional impairment are presented. Both were treated on an outpatient basis under local anesthesia via complete excision of the hyperplastic tissue and healing by secondary intention, supplemented by the application of an autologous membrane of nanofat and fibrin-rich plasma; complete skin coverage was achieved in approximately 10 days.

Discussion and Conclusions: Literature on severe rhinophyma focuses primarily on resection techniques, while reconstructive alternatives present limitations regarding fibrosis, elasticity, and aesthetic quality. In the cases presented, the use of an autologous membrane of nanofat and fibrin-rich plasma accelerated the epithelialization process and contributed to satisfactory aesthetic and functional outcomes, representing a safe, cost-effective, minimally invasive, and intraoperative alternative for reconstruction following the excision of severe rhinophyma.
