Abstract

Introduction: Davidenkow syndrome is a rare disease characterized by severe body asymmetries secondary to musculoskeletal and adipose tissue development abnormalities, with functional, aesthetic, and psychological repercussions. Fat grafting enriched with nanofat represents a reconstructive alternative with regenerative potential for volume restoration and tissue quality improvement. Objective: To present a clinical case of surgical treatment with fat grafting assisted by nanofat to correct severe body asymmetries in Davidenkow syndrome, evaluating its impact on body symmetry and its safety as a reconstructive technique and regenerative therapy. Case presentation: A female patient with asymmetry of the left breast, buttock, and leg associated with Davidenkow syndrome was treated in two surgical stages. In the first stage, fat transfer enriched with nanofat was performed in the left breast and buttock using autologous fat grafting obtained by manual liposuction. In a second stage, lipotransfer was performed on the left leg, and a contralateral periareolar mastopexy was performed for breast symmetrization. Results: Significant improvement in body symmetry was achieved, along with adequate graft integration and persistence of the transferred volume. Circumferential measurements of the leg showed a stable increase at one year of follow-up, with no evidence of significant reabsorption. No local or systemic complications occurred. Furthermore, an improvement in the texture and quality of the treated soft tissues was observed. Conclusions: Lipograft enriched with nanofat is a safe, minimally invasive, and effective reconstructive alternative for the treatment of severe body asymmetries associated with Davidenkow syndrome, providing volumetric and regenerative benefits with satisfactory functional and aesthetic results.
